With Famly's forms feature, you can send forms to individual children or multiple families at once, saving time while keeping information organised and up to date.

What to Use Follow-up Forms For
Forms can be used for a wide range of tasks, helping you stay organised and compliant. Some common examples include:
Annual data review: Parents confirm or update child and add new contact information
Dietary and allergy information: Collect up-to-date dietary requirements or allergy details
Registration paperwork: Request additional documentation such as birth certificates or vaccination records
How To Send A Form
Before you can send a form, make sure it’s been published. You can learn more about creating and managing forms in our article on Enquiry and Follow-up Forms.
Send a form to an individual child
If you only need information from one family, you can send a form directly from their profile:
Go to the child’s profile or enquiry page
Find and select the form you want to send
Choose which parent or guardian contact should receive it
Click Send form
The selected contact will receive a notification with a link to complete the form.
Send forms to multiple children in bulk
Sending forms in bulk is ideal when you need the same information from multiple families, whether that’s a group, a room, or your whole nursery. To send a form in bulk:
Go to Attendance in your sidebar
From here, open the Registration tab
Select Forms
Click the arrow icon next to the form you want to send
Choose your recipients:
Select specific children individually
Choose an entire room
Select all children at your nursery site
Click Send Form
All selected families will receive the form at once. You can view and download responses from your Forms Dashboard, or directly from each child’s profile.
Important Details About Form Links
Here’s what you need to know about how form links work:
Forms sent to unauthenticated contacts expire after 7 days
If a parent doesn’t complete the form within this time, you can easily send a new linkContacts with a Parent role will always have access to the form through their Famly app for as long as the form remains published, regardless of the 7-day expiration
